When I first started DMing, I remember players were always fighting to get some kind of surprise round on an enemy, often through cunning (or more often underhanded) ploys. But as I played more, I found that the surprise round sort of faded from the game. That extra round didn't often add up to much. It's also good to be on this side of a surprise round to avoid being caught flat footed, but I don't remember anybody from the Crescent's Edge campaign every being worried about that.
